What documents do I need to bring to my appointment for help with utilities and rental assistance?

– Picture ID on every individual in the household 18 and older
– Proof of all income (wages, disability, unemployment, child support, food stamp, etc.)
– Social security cards for every individual in the household
– Birth certificates for every individual in the household, regardless of age
– If you are separated from your spouse or have Zero Income, an affidavit must be completed and notarized. (Form will be provided by your Case Manager)
– If you are not working you will have to register with the WIN JOB Center and provide 2 job searches, if you are under the Doctor care you will need a letter on company letter head within 30 days of your appointment stating you are unable to work. If you have applied for Disability you can bring in documentation about the case, however it has to be within the 30 days of your appointment and your case must be pending.
– For Rental assistance you must provide an updated lease agreement. All private Landlords’ must complete a W-9 form (located in our office).
– For utility assistance you must provide a current bill. ALL utility bills must be in the Head of Household’s name or the Spouse; however the Spouse must reside in the home. The utility bill cannot be in a child or another relative’s name
